About The Program

At Compass of Care, we believe that expert advocacy and compassionate guidance should never be out of reach when your family needs it most. Through the Compass of Care Access Program, we offer care management and emergency advocacy services at a reduced and affordable monthly cost for seniors, individuals with disabilities, and families on Medicaid or fixed incomes. To ensure every family receives our highest standard of care, we accept 10 clients into this program per year. Once you submit the application form below, our team will reach out directly to set you up with the next steps.
